1. Our contract with you

Our Services:

We offer memorial or funeral services that includes:
Standard Memorial Service

  • Transfer of remains from place of death to LMCCI (w/in 25 kms)
  • Embalming using imported chemicals
  • Beautification using imported make-up
  • Casketing using the preferred casket type: Imported or local – wood, or metal
  • 4 days viewing or longer inside one of Loyola's Chapels or preferred viewing place by the immediate family
  • Securing relevant permits
  • Registration of death certificate, in certain case
  • Free xx copies of certified true copy of the death certificate
  • Hearse delivery to interment site
  • Funeral director and Funeral Host during interment
  • Depending on the package chosen, amenities which may include:
    • Flower (garden) setting
    • Photo/Video Services
    • Slideshow presentations
    • Balloon release during the interment
    • Throw away flowers at the interment
    • Police escorts
    • Thank You cards
    • Picture blow-up of the deceased
    • Welcome platter
    • Obituary
    • Free flowing brewed coffee
    • Flower Car
    • Family car
    • And other amenities or services that the family may require

Cremation

  • Transfer of remains from place of death to LMCCI (w/in 25 kms)
  • Cremation of the remains
  • Standard Urn or preferred urn type
  • 4 days viewing or longer inside one of Loyola's Chapels or preferred viewing place by the immediate family
  • Securing relevant permits
  • Registration of death certificate, in certain case
  • Free xx copies of certified true copy of the death certificate
  • Funeral director and Funeral Host during cremation
  • Depending on the package chosen, amenities which may include:
    • Flower (garden) setting
    • Photo/Video Services
    • Slideshow presentations
    • Balloon release during the cremation
    • Throw away flowers at the cremation
    • Police escorts
    • Thank You cards
    • Picture blow-up of the deceased
    • Welcome platter
    • Obituary
    • Free flowing brewed coffee
    • Flower Car
    • Family car
    • And other amenities or services that the family may require

We also accept inbound or outbound remains for memorial service.

In all of the scenarios above, cost would be dependent on the kind of casket the family would choose, the size of the chapel they would require and the amenities they want included in the package. Likewise, if the deceased is a Filipino senior citizen or a person with disability, the cost will be subject to the discounts mandated by law: 12% VAT and 20% senior citizen or PWD discount upon presentation of the deceased's Senior Citizen or PWD ID. Our Requirements will include:

  • Death Certificate (without the death certificate, we cannot start the embalming nor the cremation process)
  • Photocopy of the ID of deceased
  • Photocopy of the ID of next of kin who will sign the funeral service contract
  • Proof of relation (birth certificate or marriage contract) of the next of kin to the deceased
  • 20% down payment upon signing of the contract

How we will accept your order.
The immediate family member / next of kin of the deceased or an authorized family representative shall initiate the process by making a call to Loyola to pick up the remains of the deceased from its place of death. The following information shall be required from the caller:

  • Name of Deceased / Gender / Age
  • Location of the remains
  • Name of Caller
  • Relationship to the deceased and documentary proof
  • The representative waives all rights when a kin with superior right or authority intercedes
  • Landline No. & Mobile No.
  • Family Representative at the hospital / place of death
  • Contact No. (Landline No. & Mobile No.)

Once the remains have been picked up from the place of death, the immediate family / next of kin of the deceased or the authorized family representative must come to our chapel to make the arrangements and choose the memorial package preferred. Once this has been settled, the Funeral Service Contract (FSC) shall be prepared (setting out the chosen package, services and amenities), and signed by the deceased's immediate family member / next of kin or authorized family representative who shall be required to submit a photocopy of his valid ID as a competent evidence of identity. The authorized family representative shall be required to submit the authorization letter as well. He shall execute an affidavit that in the event that a representative, heir, or administrator with superior right or authority intercedes, he shall peacefully turnover the arrangement to said party.

Once the valid death certificate is submitted to Loyola, memorial service shall begin.

If the family decides not to push through with a Loyola service after the remains have been picked up from the place of death, then the family will be charged accordingly for the pick-up and transfer of the remains to Loyola Chapel.

2. Eligibility

To be eligible to enter into a contract with Loyola, the immediate family member / next of kin or authorized family representative must be over the age of 18, present proof of relationship (birth certificate or marriage contract) to the deceased, authorization letter, an affidavit or undertaking to waive rights to kin with superior authority or rights and arrange payment method at the time of completing the contract.

3. Paying for the plan

The cost of the memorial service will depend upon the memorial package and amenities chosen by the immediate family / next of kin of the deceased or authorized family representative. Full payment of the cost of the memorial service must be made one (1) day prior to the set interment or cremation date. A 20% down payment shall be required to finalize the contract.

Payment may be made in cash, credit/debit card, or via Loyola's online payment facility. Please note that if payment is made through credit card or the online facility, photocopy of the cardholder's valid identification card must be submitted to Loyola. If the cardholder is different from the person who will sign the Funeral Service Contract, a letter from the cardholder will also be required authorizing the use of his credit card to pay for the memorial service of the deceased.

No discount can be claimed retrospectively. Any discount must be deducted prior to the full payment of the contract.

For inbound remains, full payment must be made prior to retrieving the remains from the airport or seaport.

4. Cancellation / Return

Due to the nature of the transaction, cancellation or return is not possible once the contract has been signed.

5. Complaints procedure

Complaints about our services may be directed to our quality assurance team at inquiry@loyolachapels.com.

If your complaint cannot be resolved by the end of the next working day, we will write to you within five working days of receipt, and aim to resolve within 14 days. If we cannot respond to the issue within 14 days, we will keep you updated on progress

6. Other important terms

These terms are governed by Philippine law. In case of dispute parties agree that the sole and exclusive venue shall be the competent court of the City of Makati.

7. Data protection notice

Your data is important to us. By entering into a contract for a memorial service, you agree to Loyola Memorial Chapels & Crematorium, Inc, or LMCCI using your personal information and sensitive personal information for the purpose of managing this contract on your behalf.
